Ipamorelin is a synthetic pentapeptide growth hormone secretagogue (GHS) and selective ghrelin/GHS receptor (GHSR) agonist. Distinguished from earlier GHS peptides (GHRP-6, GHRP-2, hexarelin) by its high selectivity for GH release without significant effects on ACTH, cortisol, prolactin, or aldosterone at GH-stimulating doses. This selectivity makes ipamorelin the cleanest GHS available for research applications requiring isolated GH axis stimulation without confounding neuroendocrine effects.

Ipamorelin binds the ghrelin receptor (GHS-R1a) on pituitary somatotrophs, activating Gq/11-coupled phospholipase C and IP3-mediated intracellular calcium release. This synergizes with endogenous GHRH signaling to amplify GH pulse amplitude. Ipamorelin does not suppress somatostatin release (unlike GHRH) and has minimal affinity for GHS-R1a splice variants associated with cortisol and prolactin release, explaining its superior endocrine selectivity profile.
